Why does my baby press herself against the carseat buckle?

recently my baby (14 months) has been pressing herself into the carseat buckle (down there) and she stretches her legs out and grunts. she kinda holds her breath. has anyone had this experience with there children, should i be concerned....im calling the doctor tomorrow please let me know ur thoughts i will greatly appreciate it!
